Pharmaceutical care is a type of pharmaceutical practice that revolves around  “a covenantal
relationship between a patient and a pharmacist in which the pharmacist performs drug use control
functions…..governed by awareness of and commitment to the patient’s interest”

Pharmaceutical care involves the process where a pharmacist cooperates with the patient. and other
health care providers in designing, implementing and monitoring a therapeutic plan that will produce
specific therapeutic outcomes.

The basis of pharmaceutical care is responsibility and accountability to patients for the outcome of
their drug therapy.
The overall goal of pharmaceutical care is to maintain patients at the highest possible level of
functional and psychosocial well-being through optimal management of drug therapy.

Pharmaceutical care requires continuity of care between different practice settings.
